  • Copy of the child father’s passport.
  • A copy of the child father’s identification number.
  • Copy of the child mother’s passport.
  • Copy of the child mother’s identification number.
  • Copy of the child’s birth certificate.
  • The document on marriage dissolution (if any).
  • Information from the child’s father about the living in the family with the child’s mother.
  • Copies of passports of the familiar witnesses.
  • Certificate from the place of registration of the child.
